By participating in the Nanyang Technological University (“NTU”) Lee Kong Chian School of Medicine (“LKCMedicine”) iPad Scheme, I hereby confirm and accept the following Terms and Conditions:

  1. That I have chosen one (1) unit of the Apple iPad Base Model or have topped up in cash to obtain an Apple iPad Optional Model (“the iPad”) with the respective descriptions set out in the Table below;

  2. That the iPad is issued to me solely for educational purposes at LKCMedicine;
  3. That the iPad is issued to me on an “as is” basis, and I shall accept the actual state and condition of the iPad in all respects as at the date when I receive the iPad;
  4. Possession and ownership of the iPad shall pass to me only upon receipt of the full payment by LKCMedicine;
  5. That I shall take full and sole responsibility of all data stored in the iPad;
  6. That I shall not transfer (in any form) or generate profit from the iPad before my graduation without the prior written approval of LKCMedicine;
  7. That the iPad may be disposed of by LKCMedicine in its sole discretion and/or I shall have to pay the amount of the iPad Base Model iPad Base Model (i.e. $790.25 including 9% GST) if:
    • I subsequently withdraw from LKCMedicine;
    • I have failed to collect the iPad by 31 August 2024 or such other date as may be notified to me by LKCMedicine;
  8. I shall fully indemnify and hold harmless LKCMedicine from and against any and all loss, damage or expense that LKCMedicine may incur arising out of or in connection with my use of the iPad, including but not limited to any loss or misuse of data or any indirect, incidental, consequential or reputational or special loss or damage howsoever caused to or may be incurred by LKCMedicine (including payment of the legal fees and disbursement on an indemnity basis).
